Sometimes hard jerking when coming to a stop and breaking?

I recently had a wreck. Hit a telephone pole at approximately 20 MPH. Collision only to passenger side fender. Car is a 2002 Dodge Neon SXT. Now, question is: why does my car jerk roughly when I try to come to a complete stop while breaking? It usually happens when I break lightly and jerks at around 2 to 3 MPH. Doesn't jerk in reverse or acceleration, only stopping. What can and what should I do? I know a lot of family members that know a lot about cars and can fix an easy fix. Is this possibly an easy fix? It started happening after the wreck. Is it transmission problems, warped rotors? Please help I very much appreciate it. Hope I gave a sufficient amount of details. Thanks.
